Diabetes management and education

What can an APD/CDE do for you?
​
Accredited Practising Dietitians (APDs) have the qualifications and skills to modify diets and to treat diseases and conditions such as diabetes, overweight and obesity.
​
Diabetes Educators provide support for people with diabetes, including gestational diabetes, integrating clinical care, self-management education, skills training and disease-specific information to motivate patients to:
​
  • Understand the different types of diabetes (prediabetes, Type 1 Diabetes, Type 2 Diabetes, MODY, LADA and Gestational Diabetes) and make informed lifestyle and treatment choices.
  • Easily incorporate diet and physical activity into daily life
  • Educate on appropriate use and types of medicines/insulins.
  • Monitor and interpret their blood glucose pattern
 
Evidence shows that nutrition/lifestyle intervention can significantly improve patient outcomes, resulting in fewer hospital admissions and readmissions, better medical outcomes and improved quality of life. In fact, one landmark study looking at the development of Type II diabetes in people with pre-diabetes found that diet and lifestyle intervention was more effective in preventing Type II Diabetes than medication. 
 
This supports the notion that timely intervention by a dietitian/diabetes educator can reduce the risk of developing chronic disease and reduce the complications associated with these diseases.

FOR NEWLY DIAGNOSED T2D/T1D OR GDM CLIENTS, A MINIMUM OF 3 VISITS IS REQUIRED TO COVER ALL NECESSARY INFORMATION.
